Catchment Area

Step 1: Complete the required forms

Download and complete the appropriate registration forms and bring them to the surgery for submission:

Alternatively, you can collect a copy from our reception or register online via the link below.

Step 2: Provide proof of ID and address

Please bring:

  • Photo ID (passport, driving license, or birth certificate)
  • Proof of Address (bank statement or utility bill, dated within the last 3 months)
If you are registering a child under 6, please bring their red book (Personal Child Health Record) or immunisation history from previous records. This will allow us to check their immunisation status.

These documents are needed to verify your identity and to grant online access to services.

Step 3: Finalising your registration

Once we receive your completed registration form, we aim to process it within a week. During this time, please ensure you have enough medication from your previous GP to avoid running out.

Non-urgent advice: Please note

Registration is at the discretion of the doctor. If you’re outside our practice area, we may be unable to register you. In this case, we recommend choosing another GP closer to your home.

It may take several weeks for your medical records to be transferred from your previous practice.

Safe Surgery Commitment

We are proud to be a Safe Surgery.

This means that everyone is welcome at our practice, regardless of their immigration status or personal circumstances.

We will never ask for proof of immigration status or share your information with immigration authorities. Our aim is to provide healthcare to all, especially those who may feel vulnerable or face barriers when accessing medical services.

If you have any concerns or need additional support during the registration process, please speak to our team in confidence.

We are here to help.
