Step 1: Complete the required forms
Download and complete the appropriate registration forms and bring them to the surgery for submission:
- PRF1 — NHS New Patient Registration [PDF]
- Patient Registration Form (over 5 years old) [PDF]
- Patient Registration Form (5 years old and under) [PDF]

Step 2: Provide proof of ID and address
Please bring:
- Photo ID (passport, driving license, or birth certificate)
- Proof of Address (bank statement or utility bill, dated within the last 3 months)
For children under 6 years
These documents are needed to verify your identity and to grant online access to services.
Step 3: Finalising your registration
Once we receive your completed registration form, we aim to process it within a week. During this time, please ensure you have enough medication from your previous GP to avoid running out.
Non-urgent advice: Please note
It may take several weeks for your medical records to be transferred from your previous practice.
